Unsafe medication practices and medication errors are a leading cause of avoidable harm in health care across the world. A strong emphasis on patient safety helps protect patients and creates an environment where better overall care is possible. Patient safety protocols can help reduce medical mistakes and prevent adverse patient outcomes. When the goal is to help people, it seems obvious that it is important to work to protect them from unintended or unexpected harm. Medication errors occur when weak medication systems and human factors such as fatigue, poor environmental, poor technology support conditions or staff shortages affect the safety of the medication use process. Medication safety ranges from drug prescription, transcription, drug mapping, drug indenting, drug dispensing, drug administration and post administration monitoring. In addition rational use of drugs especially antibiotics, avoiding under dose/ overdose errors, avoiding delays of medication, technology support and a culture of patient safety is very important. We at Rajiv Gandhi Cancer Institute and Research Centre welcome and cordially invite you to participate in this event and get benefited from the vast knowledge and experience of the learned faculty.
